Comments:
An unannounced renewal inspection was completed on 1/28/19. There were 20 children present with two staff supervising. Five children's records and four staff records (previously reviewed on 1/16/19) were reviewed. Two emergency medications were reviewed. The inspector arrived at the school at 3:30 pm and departed at 5:30 pm. The children were observed during gym time, during snack, during table toy games, and during departures.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that all required information was documented in each child's record. Evidence: 1.The record of child #1 was missing the address for one emergency contact and the name, address, and phone number for a second emergency contact person. 2. The record of child #3 was missing two of two required emergency contacts. The names, address, and phone number of two people that can be contacted in an emergency is required for each child's record. 3. There was not a complete address for one of two emergency contacts for child #4. The city only was documented for the second emergency contact person.

Plan of Correction: The program coordinator documented the information for child #1 and #3 during the inspection. The site coordinator will get the remaining information from the parents and document it in each child's record.

Standard #: 22VAC40-185-240-D-1
Description: Based on observation, record review, and interview, the center failed to ensure when medication was available to be administered that there was a Medication Administration Trained (MAT) staff person was available at the site. Evidence: 1. There was a child observed in care from approximately 3:00 pm until 4:07 pm as observed on the sign out sheet completed by the parent. The child had an emergency medication for diabetes only in the event of unconsciousness. The medication was observed at the site. 2. There was no staff person at the site until approximately 4:00 pm when the program coordinator arrived who has the diabetic/insulin training.

Plan of Correction: The child left at 4:07 pm. The program coordinator stated that two other staff will be trained in the diabetic/insulin module for MAT in February.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, the center failed to ensure that each staff person had a completed DSS search of the central registry within 30 days of employment. Evidence: 1. The record for staff #3 was reviewed. Staff #3 had a re-hire date of 5/15/18. The original hire date for staff #3 was documented as 3/21/16. There was no completed DSS search of the central registry as a part of the background check in the record of staff #3. 2. The program coordinator stated that the search could not be located. According to the program coordinator staff #3 was terminated August 2017 and then rehired in May 2018.

Plan of Correction: The program coordinator sent the search on 1/17/19.
