Comments:
The licensing inspector conducted an unannounced renewal inspection Thursday, January 10, 2019 from 12:39pm to 3:15pm. The children were observed resting on cots for rest time. They were later observed eating their afternoon snack and going outside to play. All classrooms and playgrounds were inspected today. The facility?s van was was also inspected. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered however none is at the center. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/17/2018 Last shelter-in-place drill: 12/17/2018 (practiced monthly) Fire Inspection: 11/06/2018 Health Inspection: 11/02/2018 Total enrollment today: 14 Today, the following child to staff ratios were observed: Two to Three years old? 6:1 Four to Five years old ? 9:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records on 01/10/2019, the center did not ensure that at least every two years staff members shall obtain and submit the results of a follow-up tuberculosis (tb) screening. EVIDENCE: 1. The most recent tb screening for Staff #3 and Staff #4 is dated 10/13/2016. 2. The most recent tb screening for Staff #5 is dated 03/07/2016.

Plan of Correction: Will get health department come here or go there by 01/21/2018.

Standard #: 22VAC40-185-440-E
Description: Based on observation on 1/14/2019, the center did not ensure that there shall be at least 12 inches of space between occupied cots. EVIDENCE: In the four-five year old room, the cots that the children were resting on were not at least 12 inches apart. Some cot sides were touching.

Plan of Correction: We have a training scheduled for Saturday. Will refresh staff on nap time requirements.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 01/10/2019, the center did not ensure that over-thecounter skin products shall not be kept beyond the expiration date of the product. EVIDENCE: The following over the counter skin products were observed under the diaper changing table in the boys bathroom: 1. suncreen belonging to Child #2 that expired 11/20/2018, 2. suncreen belonging to Child #3 that expired 03/2015, 3. suncreen belonging to Child #1 that expired 08/2017.

Plan of Correction: Corrected on site. Discarded sunscreen. Will remind staff to throw out at the end of every summer.
