The licensing inspector conducted an unannounced monitoring inspection. Activities, story time, art time, games, music time, personal hygiene, lunch time, and rest time were observed. There were 35 children present, aged 10 weeks-5 years, with 8 staff, and administrative staff. Children were observed; and staff and children files and other documentation were reviewed. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 9:30am and concluded at 1:30pm.

Standard #: 22VAC40-185-160-A Description: Based on review of staff records the facility failed to maintain compliance as required with this standard relating to staff PPD/TB screening or tests. Evidence: Staff record #1, hire date 09/10/2018, did not have a TB test or screening on file. The allowed 21 days to obtain this had since expired. Plan of Correction: A copy of the staff members most recent TB test will be obtained and placed within the file for future compliance and review. If a copy cannot be obtained the staff member will get a new TB test or screening as soon as possible. New staff will be required to have a recent TB test and proof of the findings prior to hire moving forward.
Standard #: 22VAC40-185-60-A Description: Based on review of children records the facility failed to maintain compliance as required with this standard related to proof of birth verification. Evidence: Child record #2, enrollment date 01/14/2019, did not have verification of proof of birth as required. The allowed 7 days to verify this had since expired. Child record #3, enrollment date 06/11/2018, did not have verification of proof of birth as required. The allowed 7 days to verify this had since expired. Plan of Correction: Proof of birth verification will be documented as required for both children. Administrative personnel will continue to verify that a record is completed in it's entirety upon enrollment and that any updates are noted for future and continued compliance.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records the facility failed to maintain compliance as required with this standard related to staff references. Evidence: Staff record #5, hire date 2017, had only 1 of the required 2 references on file. Plan of Correction: The second reference will be located, or another reference will be obtained for compliance. Prior to hire of new staff references will continue to be obtained as required.
