Comments:
A Subsidy Health and Safety inspection was conducted on January 29, 2019 to review supplemental health and safety requirements. Eleven staff were present with eighty-one children. Additional staff were on site for administrative duties. Eight children's records and seven staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on review, facility did not ensure 1 of 8 children's records had documentation of required information related to food allergies. Evidence: On January 29, 2019, the record for child #8 had documentation of a diagnosed food allergy noted on the physical form and there was not a written allergy care plan from the physician stating the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: Parent will be contacted to get updated documentation from physician.

Description: Based on interview, facility did not ensure the emergency preparedness plan has required written procedures. Evidence: On January 29, 2019, the emergency preparedness plan did not have a policy for the continuity of operations to ensure that essential functions are maintained during an emergency.

Plan of Correction: Policy was written and printed during inspection.
