Comments:
An unannouced monitoring inspection was conducted beginning at 6:00 and ending at 7:20. As of 7:00, there were 14 children in care with 4 staff. Children were observed upon arrival and participated in small group table activities. Reviewed 4 staff files - complete. Reviewed medications for 4 children - complete Reviewed records for 8 children - see violation notice Emergency drills are up to date as are Health and Fire Inspections. Description: Based on a review of 8 children's records, it was determined that not all reviewed records are complete. Evidence: The record for Child #3 is missing complete addresses for 2 emergency contacts and parent agreements (Parent Pledge); the records for Child #6 and #7 are missing physical and shot records; the record for Child #8 is missing one emergency contact other than a parent.

Plan of Correction: The Director will review all files for complete information, notify parents and complete all files.
