A renewal inspection was conducted today from 10:00 am to 12:00 pm. There were 58 children and 15 staff present. Children were observed having tummy time, napping, reading books in circle time, painting, building with blocks, painting the planets, and playing with farm animals. Reviewed 6 staff files, 3 children's files, fire and health inspections, emergency evacuation log, menus, attendance, medications/topicals, and physical plant. Violations of the Standards are recorded on the Violation notice. Standard #: 22VAC40-185-60-A Description: Based on record review, a child's record did not contain documentation of child updates and confirmation of up-to-date information. Evidence: Child#1 did not have documentation that a annual review of information was done, and confirmation the child's information was up to date. Last signature indicating information was current was 12/6/17. Plan of Correction: I will have the parent update the info and sign off.
Standard #: 22VAC40-185-270-A Description: Based on observation, equipment in the center was not maintained in a safe condition. Evidence: Soft play equipment in the Toddler room was observed to be frayed and had exposed stuffing, which is a potential health and choking hazard. Plan of Correction: We will repair, remove or replace the equipment.
Standard #: 63.2(17)-1721.1-A Description: Based on record review, an agent of an application for licensure as a child care center did not obtain any of the three required background checks. Evidence: Agent #1 did not have a signed Sworn Statement Disclosure, nationwide Fingerprint Clearance letter or Central Registry Abuse and Neglect Clearance letter as required. Plan of Correction: I will make sure these get done this week.
