An unannounced non-mandated monitoring inspection was conducted to assess the correction of previous violations. There were 43 children in care of 12 staff. 5 children's and two staff records were reviewed. The children were observed finishing lunch, listening to stories and preparing for nap time. Emergency drill records were current. Areas of non compliance are listed in the violation notice. Standard #: 22VAC40-185-140-A Description: Based on record review, each child enrolled did not have a physical exam within 30 days of enrollment. Evidence: Child # 1's date of enrollment was 7/17/17. Documentation of a physical exam was not obtained for Child # 1. Plan of Correction: WE will ask the parents to bring in a copy of the physical exam.
Standard #: 22VAC40-185-160-C Description: Based on record review, the center did not obtain a subsequent tuberculosis (TB) screening every two years for staff. Evidence: Staff # 2's most recent TB screening was dated 1/11/17 ad expired on on 1/11/19. An updated TB screening was not obtained. Plan of Correction: I will send my staff to get the updated screening.
Standard #: 22VAC40-185-60-A Description: Based on records review, the center did not annually children's records. Evidence: 1. Child # 1's first day of enrollment was 7/17/17. An annual update of information was done for Child # 1. 2. Child # 2's record indicated that the most recent update was done in August 2016. 3. Child # 3's first day of enrollment was 6/9/17. An annual update of information was done for Child # 3. Plan of Correction: We will complete the annual updates.
Standard #: 22VAC40-185-570-I Description: Based on observation and staff interview, a one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care was not maintained at the center. Evidence: 1. The center did not maintain an emergency supply of extra bottles, nipples and commercial formula for infants. 2. Staff # 1 stated that there was some formula in the facility that could be used for the infants in case of an emergency. Plan of Correction: We will work on maintaining the emergency supplies.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review, the center did not obtain fingerprint results for staff before the first day of employment. Evidence: 1. Staff # 3's first date of employment was 10/10/18. 2. Staff # 3's fingerprint results were not obtained before the first day of employment. They were dated 10/16/18. Plan of Correction: We will make sure that we obtain results before the first date of employment.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review, the center did not obtain an out of state central registry for an employee that has resided outside of Virginia in the last 5 years. Evidence: 1. Staff # 3's first day of employment was 10/10/18. Staff # 3's out of state central registry was mailed on 10/9/18. 2. At the end of the 45 day period, the center did not conduct a follow up regarding Staff # 3's results and they were not available for review during the inspection. Plan of Correction: We will call and follow up.
