Comments:
An unannounced monitoring inspection was conducted today. There are 13 children present at LI arrival with 2 staff. 2 staff records were reviewed for specific items only. The entire staff files is reviewed at another time at the main office. 2 children's records were reviewed. See violation notice for missing items. Children were observed participating in a variety of activities in the room. The playground was not in use during today's inspection. Emergency escape drills, first aid supplies, emergency supplies are all in compliance. LI was present today from 3:25 PM -4:40 PM

Description: Based on a review of medications, authorization forms are missing a required component. Evidence: 5 of 6 authorizations reviewed were missing a parent signature. (children 1,2, 3)

Plan of Correction: I will ask the parents to sign the forms.
