Comments:
An unannounced monitoring inspection was completed on 2/4/19. There were 28 children present with three staff supervising. Five children's records were reviewed; three staff records had been reviewed previously and will be reviewed for updates at a later date. There were no medications at the center. The inspector arrived at the school at 2:30 pm and departed at 5:00 pm. The children were observed during arrivals, during snack, and during outside time. There was discussion with the site coordinator about the following: having the board with all required posted information in the cafeteria, school cleaning supplies in the cafeteria that may need attention, checking cleaning bottles to ensure the label does not wear off.

Description: Based on observation, the center failed to ensure that hand washing was practiced as required. Evidence: 1. The children were observed during snack. There were several (at least six of the approximate 20 children who were eating snack) observed eating snack and not washing their hands or using disposable wipes after eating.

Plan of Correction: The site coordinator will make sure that all children wash their hands or use disposable wipes after eating daily.
