Comments:

The licensing inspector conducted an unannounced renewal inspection on Friday, February 8, 2019 from approximately 9:47am to 12:45 pm to determine the center's compliance with licensing standards. During this inspection, the children in care were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 46 children with 10 staff present including administrative staff. The licensing inspector completed observations of the children in care completing a variety of activities to include engaging in free play, participating in centers, working on various heart activities, feedings, playing outside at the playground areas, and eating lunch. The children's lunch today consisted of fish sticks, corn, yogurt, and milk. The center's bathrooms, diapering station, designated classrooms and playground areas were inspected by the licensing inspector. The required posting were reviewed by the inspector. During the inspection there were six children?s and five staff's records reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last fire inspection: 03/26/2018 Last health inspection: 11/09/18 The Department has received a complete renewal application and annual fee from the center. Standard #:	22VAC40-185-140-A
Description:	Based on review of children's record, the center did not ensure to obtain a physical examination for each child. Evidence: During the inspection on February 8, 2019, there was no documentation of a physical examination in the records for Child#3 (first date: 07/30/18) and Child#6 (first date:05/07/18)
Plan of Correction:	The center will request the required information from the parents today and will give them until February 16, 2019 to return to the center.
Standard #:	22VAC40-185-60-A
Description:	Based on review of children's record, the center did not ensure to obtain all required information. Evidence: During the inspection on February 8, 2019, in Child#2's record (first date: 01/03/18) there was no documentation of viewing proof of the child's identity and age.
Plan of Correction:	The center will contact child's mother today to request birth certification to be provided by Monday, February 11, 2019.
Standard #:	22VAC40-185-270-A
Description:	Based on observation, the center did not ensure all areas and equipment were maintained in a clean, safe, and operable condition. Evidence: During the inspection on February 8, 2019, there were three pieces of ride on play equipment cracked in multiple area which a sharp edge located on the playground while children were actively engaged in play.
Plan of Correction:	This was corrected during the inspection. The staff will be reminded of the playground procedures on Monday, February 11, 2019
Standard #:	63.2(17)-1720.1-B-2
Description:	Based on review of staff records, the center did not ensure to submit fingerprinting to obtain the proper OBI determination letter for all staff. Evidence: During the inspection on February 8, 2019, a review of Staff#2's (DOH: 06/15/17), revealed a OBI fingerprinting determination letter was not submitted for this specific center's site.
Plan of Correction:	An OBI fingerprinting for this staff was completed in the summer but a copy was not maintained in the record. The center will contact OBI on Monday, February 11, 2019.
