Comments:
An unannounced Monitoring inspection was conducted in the center on February 7, 2019. The Licensing Inspector observed 75 children in care with 19 staff members supervising their care and activities. The children were observed in 8 classrooms. Medications were observed in the Center. A sample of 10 children's records was reviewed. Documentation of the Center's annual fire and health inspections was current. Documentation of the practiced emergency drills was reviewed. Information was observed for 4 staff, partial records were reviewed; complete staff records are maintained at the central office. Staff were observed interacting with the children. Staff were observed reading books to the children, a staff member was engaged at the sand table with children, several groups were taken outdoors for play and staff was observed during an art activity with children. Findings of the inspection were reviewed at exit with one of the designated Lead. Standard #: 22VAC40-185-500-B
Description: Based on direct observations, the Center failed to ensure staff followed proper procedures during diaper disposal as defined in standard 500.B.6., Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during the disposal. Evidence: The LI observed diapering procedures in the infant room. A staff member was observed during several diaper changes. The staff member used her hand to open the lid to the storage can to dispose of the paper barrier and soiled diaper; the foot pedal was not used by the staff member.

Plan of Correction: Diapering procedures will be reviewed with the responsible staff. Staff will be monitored to ensure center's procedures are followed.

Description: Based on direct observations, the Center failed to ensure tables were sanitized before and after each use for feeding Evidence: The LI observed as staff and children prepared for the lunch meal in classroom F. Prior to placing the food bowls on the table, the children were seated at the table with individual drawing pads. Once the food cart arrived to the room, staff instructed the children to put their activity away and wash their hands. Staff immediately placed the meal on the table, the table was not sanitized prior to being used for the meal.

Plan of Correction: The center's procedures for meal/snack times will be reviewed with staff. Staff will be monitored to ensure procedures are followed.
