Consultation was given on documentation for drills and injury reports.

An unannounced monitoring visit was conducted today from 9:30-10:40. Five children were present with two adult caregivers. Observed transition into the program, free play, building with magnetic tiles, coloring and listening to music. One emergency medication is on-site with a staff having current certification in Medication Administration Training (MAT). Room is well organized and equipped for preschool children with ample supplies and materials. Snack is bought from home. Areas of non-compliance are identified in the violation notice. Description: Based on record review, 2 of 3 children's records are incomplete. Evidence: Child #1 and Child #2 are missing addressed for 2 emergency contacts. Child #1's allergy action plan can not be followed through as one of the emergency records is not on-site.

Plan of Correction: Will contact the parent for information and update the action plan as needed.

Description: Based on record review for staff, 2 of 3 records are incomplete. Evidence: Staff #2 and Staff #3 are missing a name, address and telephone number for an emergency contact.

Plan of Correction: Will request paperwork back from the administrative office or complete new ones as needed.

Description: Based on observation, hazardous substances are not kept in a locked cabinet preventing access by children. Evidence: The upper cabinet containing bleach and cleaning solutions was unlocked. The key is with the staff member who was on leave today.

Plan of Correction: Cabinet will be locked when not in use.

Description: Based on observation, an emergency medication is not properly labeled. Evidence: The medication is not in the original container/box with the child's name, dosage amount and time to be given.

Plan of Correction: The center staff will request the parent bring in the medication box with the prescription label.

Standard #: 22VAC40-185-540-C
Description: Based on observation, the first aid kit is incomplete. Evicence: No thermometer is present in the first aid kit or in the classroom.

Plan of Correction: A new one will be ordered for the program.

Standard #: 22VAC40-185-540-E
Description: Based on staff statement, non medical first aid supplies were not found. Evidence: A flashlight and battery operated radio were not found during the inspection.

Plan of Correction: New ones will be ordered and placed with the first aid kit.

Standard #: 22VAC40-185-550-M
Description: Based on observation and staff statement, an injury report was incomplete. Evidence: Injury report (11/1/2018) did not include either 2 staff signatures or a staff and parent signature.

Plan of Correction: Will ensure that any new injury reports are filled out completely.
