Technical Assistance:

Comments:
An unannounced monitoring inspection was conducted on 2/12/19 from 10:00am - 12:15pm. During the inspection there were 58 children ages two years old through five years old in care with 15 staff. Children were observed participating in various activities in the classrooms and eating snack. Records were reviewed for six children and eight staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. A license modification request was made by the facility to expand the current licensed program to include the before and after school program, the school age and preschool summer camp, and the extended day preschool program, and eliminate the religious exemption for these programs. The areas and equipment used for these programs were inspected. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of six children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #5 did not contain documentation for second person to contact in an emergency if the parent can't be reached.. 2. Staff #7 (Assistant Director) confirmed that the record for child #5 did not contain all of the required information.

Plan of Correction: The facility responded: The parents of child #5 will be asked to complete the missing information. All newly enrolled children will have their records reviewed at enrollment to ensure they are complete.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of eight staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #5 (date of hire 2/6/18) did not contain documentation of a completed search of the central registry finding. 2. Staff #6 (Program Director) reviewed the record for the staff #5, and confirmed that the search of the central registry finding has not been received.

Plan of Correction: The facility responded: A new search of the central registry for staff #5 was completed and mailed. During the inspection, the office of Background Investigation was called and said the search of the central registry for staff #5 was being returned because it was not legible. Staff #5 will complete a new search of the central registry. All new staff will complete upon employment. If it is not received within 30 days of employment, we will call the Virginia Department of Social Services to check the status of the request.
