Comments:
A self reported incident involving care and protection of one child was investigated on 01/25/2019. Observation of the center's video recording of 01/21/2019, a review of staff and child records and written staff statements were reviewed and both staff and a parent interview was conducted. During the investigation, a violation was determined pertaining to care and protection. Other violations which were not reported were also found during the investigation. Standard #: 22VAC40-185-340-A
Description: Based on the center's video recording, staff interviews and staff written statements, staff failed to ensure the care and protection of a child. Evidence: According to the Center's video recording on 1/21/19, Staff #1 placed a sleep sack blanket over Child #1's head while Child #1 was in a crib. Staff #1 replaced the sleep sack blanket over Child #1's head on 3 separate occasions during a period of 23 minutes.

Plan of Correction: Staff were retrained on the care and protection of a child. Staff will receive additional infant training on 2/18/19

Standard #: 22VAC40-185-430-B
Description: Based on the center's video recording and staff interviews, equipment was used that was not age and stage appropriate. Evidence: According to the center's video recording on 1/21/19, Staff #1 placed Child #3 (8 months old) in a bouncy seat. Child #3 is able to sit independently. The manufacturer's instructions for this equipment is for children under the age of 6 months and for those children who can not sit up independently.

Plan of Correction: Staff were retrained on appropriate equipment based on age and stage. Additional training will be provided on 2/18/19.

Standard #: 22VAC40-185-500-A
Description: Based on observation of the center's video recording system, hand washing procedures were not followed. Evidence: According to the center's video recording on 1/21/2019, Staff #1 failed to wash their hands and the hands of two children (Child #2 and #3) before and after eating. Staff #1 did not use the step on device to raise the lids of the trash and diaper receptacle, lifted the lid on both the trash receptacle and diaper receptacle with their hands, and did not wash their hands after doing so.

Plan of Correction: Staff were retrained on handwashing procedures. Staff will receive additional training on hand washing on 2/18/19

Standard #: 22VAC40-185-560-I
Description: Based on observation of the center's video recording, food was not prepared and stored in a sanitary manner. Evidence: According to the center's video recording of 1/21/2019, Staff #1 was feeding Child #2 and Child #3 while the children were sitting in a booster/tray chair and a bouncy seat. Staff #1 placed a bowl of food on the floor on two occasions while feeding the two children.

Plan of Correction: Staff were retrained on food preparation and the storing of food in a sanitary manner. Staff will receive additional training on 218/19.

Standard #: 22VAC40-185-560-K
Description: Based on the Center's video recording system, a high chair tray was not sanitized properly. Evidence: According to the video recording on 1/21/19, Staff #1 did not sanitize the tray used by Child #2 before and after Child #2 ate.

Plan of Correction: Staff were retrained on proper sanitizing procedures. Additional training will be provided on 2/18/19

Standard #: 22VAC40-185-570-A
Description: Based on observation of the Center's video recording, a high chair, infant carrier seat or feeding table was not used. Evidence: According to the center's video recording on 1/21/19, Staff #1 placed Child #3 (age 8 months) in a bouncy seat and fed Child #3.

Plan of Correction: Staff were retrained on appropriate feeding equipment. Additional training will be provided on 2/18/19.
