Comments:
An unannounced mandated inspection was conducted from 10:30 until 1:00. There were 16 infants, toddlers, and preschoolers in care with staff maintaining ratios at all times. Infants and toddler were observed playing, sleeping in cribs, and being changed. Preschoolers were observed playing dress up, eating lunch, and during rest time. No medications was reported to being administered. Five children's records and five staff records were reviewed. The violation from today is identified on the violation notice. The violations from last time had been corrected. Please call your inspector at 276-698-9981 with any questions.

Description: Based on record review and interview, the center did not have a central registry finding within 30 days of employment. Evidence: 1. The inspector reviewed five staff records. One of five records did not contain a central registry finding on a staff person within 30 days of employment. Staff # 3 was hired 9/14/2018 and the central registry had not been returned.

Plan of Correction: Staff called the OBI office during the inspection do check on the status, it will be reprocessed.
