Description: Based on record review and interview, the center did not ensure that staff members shall obtain and submit the results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing. Evidence: 1. The most recent tuberculosis test for staff 1 is dated 12/21/16. An updated test was due in December 2018. 2. The center director confirmed that the tuberculosis test located in staff 1's record is expired.

Plan of Correction: Staff 1 stated that she obtained an updated TB test in 2017 and will contact her physician to obtain a copy, which will be placed in staff 1's record.

Description: Based on record review and interview, the center did not ensure that each child's record contains all the required information. Evidence: 1. The records for child 1 and child 2 do not contain the addresses of two designated people to call in an emergency if a parent cannot be reached. 2. The center director confirmed that the records for child 1 and child 2 do not contain the addresses of the two required emergency contacts.

Plan of Correction: The center director will request that the parents of child 1 and child 2 provide the addresses for the required emergency contacts, which will be placed in the children's records.
