An unannounced monitoring inspection was conducted on February 22, 2019. There were 110 children present at arrival to the facility in eight classrooms with 15 staff members. Additional staff members and volunteers were on site for administrative and support purposes. The children were observed having lunch which consisted of chicken teriyaki, green beans, bread, peaches, breadsticks and milk. The children were also observed washing their hands, napping, reading books and playing quietly on their cots if they were not napping. The outdoor playground area was not inspected due to inclement weather. Ten children's records were reviewed. Fourteen medications were reviewed.

Standard #: 22VAC40-185-140-A Description: Based on record review, in one of ten children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician: before the child's attendance or within one month after attendance. Evidence: During the inspection conducted on February 22, 2019, there was no documentation of a physical examination in the record for Child #1 (start date 10/29/18). Plan of Correction: The case manager has been contacted and made aware that the physical is needed ASAP. The case manager will contact the parent and the Health Coordinator will be contacted as well.
Standard #: 22VAC40-185-270-A Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center shall be maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on February 22, 2019, in Room 110 there were multiple areas of chipped and peeling paint observed on the wall, cabinet, bathroom door and a heating unit in areas that were accessible to the children in care. Plan of Correction: We will alert the custodians today that those areas need to be painted in Room 110.
