The licensing inspector conducted an unannounced renewal inspection on 02/22/2019. The children were observed playing outside, eating lunch and preparing for nap. A formal observation was completed with the preschool and toddler/twos classroom. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and the center's mini bus was inspected today. Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Three injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 01/30/2019 Last shelter-in-place drill: 11/16/2018 Last fire inspection: 04/19/2018 Last health inspection: 11/30/2018 Today, the following child to staff ratios were observed: Toddlers / Twos ? 13:2 Preschool ? 8:2 The violations from the previous inspections were checked for corrections. Description: Based on record review, the center did not ensure that one of five children's records contained documentation of immunizations before the child can attend the center. Evidence: The record of child #3 (start date: 6/19/18) contained documentation of immunizations dated 6/29/18.

Plan of Correction: Make sure all forms are in file on or prior to first day of attendance.

Standard #: 22VAC40-185-180-D
Description: Based on record review, the center did not ensure that staff who drive a vehicle transporting children disclose any traffic violation that occurred prior to or during employment or assignment as a driver. Evidence: The record of staff # 1 (start date: 11/13/18) did not have documentation of disclosing their driving history prior to employment. The center's representative informed the inspector that staff #1 regularly transports children.

Plan of Correction: Will make sure driver signs violation disclosure and always prior to hire.

Standard #: 22VAC40-185-350-C
Description: Based on observation, the center did not ensure that when children are regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group applied to the entire group. Evidence: The center's toddler / two year old classroom had two staff members supervising thirteen children. There were toddler and two year old children in the classroom. The required staff-to-children ratio for the classroom was 1:5.

Plan of Correction: Make sure staff knows their assigned group at all times.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the center did not ensure to follow the most recent, age-appropriate nutritional requirements of a recognized authority. Evidence: All meal components were not offered to the children for lunch. The children were offered bread, chicken nuggets, fruit, beans, and juice. The children were not offered milk as required by USDA standards.

Plan of Correction: Make sure all meal components are offered daily.
