63.2(17) License & Registration Procedures
- Technical Assistance:
An unannounced monitoring inspection was conducted on Friday, March 1, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 9:15 am and concluded the inspection at 12:30 pm. The center?s assistant director assisted the inspector throughout this inspection. The census for today?s inspection consisted of 60 children in the direct care of 11 staff. Upon the inspector's arrival, the children were engaged in a variety of activities. The children in the Crickets classroom were formally observed during snack time and during a teacher-led activity. The children were playing a syllable count game and were observed interacting positively with their peer and staff. The children in the June Bugs room were observed playing in different centers and engaged in a one-on-one art activity with a staff member. Diapering and hand-washing procedures were observed in another classroom. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. Medication is not administered at this time. There are multiple staff members with CPR/FA training, and multiple MAT certified staff. Six children's records and six staff records were reviewed during today's inspection. Standard #: 22VAC40-185-160-A Description: Based on a review of six staff records on 03/01/2019, the center did not ensure documentation for one staff member's tuberculosis screening (TB) was completed within 12 months prior to employment. Evidence: 1) The record for Staff #1 (DOH: 11/05/2018) contained a TB screening result dated 08/31/2017, which is more than 12 months prior to the first day of employment. Plan of Correction: Per the Center: The staff will complete another TB test by Monday. In the future, the center will make sure the TB test is dated within 12 months of their hire date.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of six staff records on 03/01/2019, the center did not request a search of out-of-state founded complaints of child abuse for one staff member who has lived outside of Virginia within the last five years. EVIDENCE: 1) During the inspection, the Sworn Statement for Staff #3 indicated the staff member had lived outside of Virginia within the last five years. The records did not contain documentation of a requested founded complaint of child abuse from the state the staff member indicated he had lived in within the last five years. 2) During interview, a member of management confirmed a search of out-of-state founded complaints of child abuse had not been requested for Staff #3. Plan of Correction: Per the Center: The center will submit the out-of-state request for Staff #3 by Monday. In the future, the center will follow up with out-of-state requests in a timely manner.
