An unannounced monitoring inspection was conducted from 10:20 am-12:40pm. 40 children were in care with 12 staff members present. Children were observed engaged in craft activities, outdoor play time, being bottle fed and having lunch. The lunch menu included roast pork loin, halal beef and seared tofu, steamed rice, buttered sweet corn, salad and milk. 4 staff and children records, emergency drill records were reviewed. Standard #: 22VAC40-185-160-A Description: Based on staff interview, the center did not obtain a negative tuberculosis (TB) screening from each staff member. Evidence: 1. Staff # 1 stated that the Pre-K 4 class attends curriculum related class with Staff # 2 and Staff # 3 in another part of the campus. 2. Staff # 2 and Staff # 3 are not full time employees at the center. 3. The center did not obtain TB screening results for Staff # 2 and Staff # 3 and they were not available for review. Plan of Correction: I will obtain the TB screenings by the end of the week.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment in the center were not maintained in a safe condition. Evidence: 1. Peeling paint was found on the walls in the Pre-K 3 and Toddler room. 2. A stuffed toy elephant in the toddler room had a hole under the trunk, exposing the inside stuffing to the children. Plan of Correction: A work order will be place for the paint. We will fix the hole in the elephant.
Standard #: 22VAC40-185-550-D Description: Based on record review, the center did not an evacuation drill at least once a month. Evidence: The fire drill log indicated that the most recent fire drill was conducted on 12/19/18. There was no documentation to show that an evacuation drill was conducted for the months of January and February 2019. Plan of Correction: We will conduct make up drills this week.
Standard #: 22VAC40-185-550-M Description: Based on record review, the center did not maintain complete written records of children's injuries. Evidence: 1. 6 injury reports were reviewed. 2. 1 of the 6 reports did not have the name of the child recorded. 3. 4 of the 6 reports did not have the time of notification recorded. Plan of Correction: We will discuss this in our staff meeting and retrain staff.
