22VAC40-185 STAFFING AND SUPERVISION.
The licensing inspector conducted an unannounced inspection on 02/08/19 in response to a self-report that was received in the licensing office on 02/05/19 regarding allegations in the area of supervision. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the report. Violations were cited related to the report.
Standard #: 22VAC40-185-340-A Description: Based on interviews with staff, documentation and a self report, the center did not ensure the care, protection and guidance when supervising children. Evidence: On February 4, 2019 at 4pm, Staff #1 took the Pre-K1 class outside on the playground. Staff #1 did not follow the center's policy of documenting the children's transitions on a "Name-To-Face" sheet. As a result, four year-old child #1 was left unsupervised in the classroom of the center for about 2 minutes before being discovered by the Program Director. When the Director took child #1 outside, Staff #1 reported she was unaware child#1 was not outside. Plan of Correction: The following corrective action was provided by the program director during the interview: The child was taken outside to the staff member. The child's parent was called. The staff #1 was written-up and demoted to an assistant. Additional training will be provided.
Standard #: 22VAC40-185-340-F Description: Based on interviews with staff, documentation and a self report, the center did not ensure children under 10 years of age were always in sight and sound supervision of staff. Evidence: On February 4, 2019, four year-old child #1 was left unsupervised in the Pre-K 1 classroom for about 2 minutes after the child's Pre-K1 class went on the playground. Child #1 was discovered by the Program Director who was making rounds to each class around 4:00pm. During her rounds, the Program Director confirmed seeing Staff #1 take the Pre-K1 class out the back door to the playground. Two minutes later, after exiting the last classroom, the Program Director reported seeing child #1 in the Pre-K 1 classroom, by himself with his jacket on. Plan of Correction: The following corrective action was provided by the program director during the interview: The child was taken outside to the staff member. The child's parent was called. The staff #1 was written-up and demoted to an assistant. Additional training will be provided.
