The inspector conducted an unannounced monitoring inspection from 10:00 a.m. until noon. There were 14 children in care with three and at times four staff supervising at all times. Two children were taken out of the class for services. Children were observed playing at centers, washing hands, being read to, lining up and going to and eating lunch. They were also observed doing gross motor skill activities after lunch before preparing for lunch. No medications were reported to be on site and/or being administered. Five children's records were reviewed and one new staff record was reviewed for background checks. The violation is identified on the violation notice. Standard #: 22VAC40-185-330-B Description: Based on interview and measurement, the center did not have the required resilient surfacing on the playground. Evidence: 1. The inspector measured the mulch and it was 2-4 inches deep. 2. The staff acknowledged that even though they rake it is not the required depth of 6 inches. Plan of Correction: Staff states that the main office is aware of the need for mulch and that it has been ordered.
