A Subsidy Health and Safety Inspection was conducted on March 7, 2019 from approximately 9:55am to 1:00pm to review supplemental health and safety requirements. Upon arrival to the facility, there were 41 children present with seven staff members. Additional staff were on site for administrative and support purposes. The following items were reviewed as a part of the inspection: indoor and outdoor areas, supervision, administration, health and safety, physical plant, required documentation, and emergency supplies, procedures and required postings. Three medications were reviewed. The center does offer transportation services. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-540-A Description: Based on record review, in one of five staff records reviewed, the vendor did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: The TB documentation submitted for Staff #3 (date of hire 12/19/18) was dated 10/31/18 and therefore not completed within the last 30 calendars of the date of employment. Plan of Correction: The staff member will submit an updated TB screening.
