Discussion of the incident that occurred on February 15, 2019 involving Child #1 and Staff #2, and Staff #3.

Comments:
The following unannounced complaint inspection took place on 03/05/2019, between the hours of 3:30 pm through 6 pm, on site at the Kiddie Academy located in Leesburg, Virginia. Standard #: 22VAC40-185-40-E
Complaint related: Yes
Description: Based on review, the operational responsibilities of the licensee did not ensure that the center's activities were maintained in compliance with these standards, and the center's own policies & procedures that are required by these standards. Evidence - 1. On the date of inspection (03/05/2019) a document entitled Supervision Policy was observed to be signed by both Staff #2 (dated - 07/11/2018), and Staff #3 (dated - 01/08/2019). 2. This document outlined the polices that Kiddie Academy employees operate under, stating: ". . . two-year olds must be supervised by sight and sound at all times." 3. Due to Child #1 being left unsupervised for a period of 2 to 3 minutes, this center policy was not followed on February 15, 2019 by Staff #2 and Staff #3. 4. This standard was previously cited during a Non-Mandated Inspection (NMI) conducted on June 7, 2018 for an incident that originally occurred on May 31, 2018 involving another child under 10 years of age who was also left unsupervised in a classroom.

Plan of Correction: Staff #2 and Staff #3 were both retrained on February 15, 2019 with regards to supervision, and administrative action was completed.

Description: Based on interview and review, staff did not always ensure care and protection of a child in their care. Evidence - 1. On the date of inspection (03/05/2019), it was determined that on February 15, 2019 that a two year old child was left unsupervised, and alone in their classroom for a period of 2 to 3 minutes. A name to face count of all children was not conducted prior to leaving the Monkey's classroom. 2. Both Staff #2, and Staff #3 did not follow center policy and conducted the name to face as they arrived at the playground. 3. This incident marks the second supervision issue for children under 10 years that the center has recorded since another incident involving the Penguin room occurred previously on May 31, 2018.

Plan of Correction: Staff #2 and Staff #3 were both retrained on February 15, 2019 with regards to supervision, and administrative action was completed.

Complaint related: Yes
Description: Based on interview, a child under 10 years of age was not always within actual sight and sound supervision of staff. Evidence - 1. Staff #1 stated (during an interview conducted on the date of inspection) that on February 15, 2019, approximately between 10 am to 10:30 am, a two year old child was left unsupervised within the "Monkeys" room for approximately 2 to 3 minutes. 2. It was determined that Staff #2 and Staff #3 failed to perform a name to face child count prior to leaving the classroom. Instead, the name to face was conducted prior to entering the playground at the end of the hall, away from the classroom. 3. Staff #2 stated (during an interview conducted on the date of inspection) that after the name to face was conducted (prior to entering the playground), Staff#2 and Staff #3 both realized that they were short one child. Staff #2 went back to the room to find Child #1 on the couch, unsupervised. Staff #3 also confirmed that Staff #2 brought Child #1 back down the hall to the rest of the group of 13 children. 4. This standard was previously cited during a Non-Mandated Inspection (NMI) conducted on June 7, 2018 for an incident that originally occurred on May 31, 2018 involving another child under 10 years of age who was also left unsupervised in a classroom.
