An unannounced renewal inspection was conducted March 8, 2019 to determine the facilities compliance with licensing standards. The inspector arrived at 9:00 am and concluded the inspection at 11:30 am. Upon the inspector's arrival, children were returning form the cafeteria where they had had breakfast. Once in the classroom, children listened to a story about the sun, had bathroom break, washed their hands and participated in a moment of meditation followed by stretching exercising. Later children worked in centers consisting of art (making rain clouds), construction with blocks and plastic animals, sensory table, store (with a cash register and paper money) and yoga. Staff assisted the children with the yoga, showing flashcards of different poses and later danced to music. Staff interactions were positive. Children also created a book about the different types of weather. One child read her book to the inspector. The classroom has age and stage appropriate materials and with children's work displayed. All required information was posted to include the license, last inspection notice, daily schedule, emergency numbers, menu, evacuation procedures/maps and parent information. The first aid kit and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Four staff records were reviewed along with five children's records. The records for five board officers were also reviewed. One staff member has current MAT. Staff have current CPR, first aid training and daily health observation training. Medications are on site and were reviewed. The most recent evacuation drill was conducted on 02/26/2019. The two required shelter-in-place drills for the 2018-2019 school year were completed on 09/25/2018 and 10/18/2018. Today's lunch consisted of chicken patty on a bun, broccoli with cheese sauce, carrots and dip, fruit and milk. Health Inspection: 12/19/2018 Fire Inspection: 08/30/2018 Today's Ratio: 15 children with two staff. Violations cited during the licensing period were reviewed for corrections. There were no repeat violations cited during today's inspection. A complete renewal application has been received by the department. Standard #: 22VAC40-191-40-D-1-A Description: Based on review of five board officer records, the facility did not obtain a sworn statement for a newly designated agent within the required time frame. Evidence: Board officer #3 was appointed on 10/25/2018. The documented sworn statement was dated 01/18/2019. Sworn statements for newly appointed board officers are required to be obtained within the first 30 days of being appointed. . Plan of Correction: Per the administrator: We will use the same systematic systems we use to process background checks for our employees. Background checks will be obtained before officers are appointed to the board to ensure they are completed within the time requirements.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on review of five board officer records, the facility did not obtain a fingerprint based national background check within the required time frame. Evidence: 1. The fingerprint based national background check for board officer #2 was dated 01/28/2019. Board officers that were appointed prior to 01/21/2018 were required to obtain the fingerprint based national background check by 09/30/2018. 2. The fingerprint based national background check for board officer #3 who was appointed on 10/25/2018) was dated 01/30/2019. Fingerprint based national background checks are required to be obtained within the first 30 days of being appointed. Plan of Correction: Per the administrator: We will use the same systematic systems we use to process background checks for our employees. Background checks will be obtained before officers are appointed to the board to ensure they are completed within the time requirements.
