Comments:
An unannounced Monitoring inspection was conducted in the Center on March 6, 2019 from 9:10 to 11:40 AM. Reportedly, there are 20 children currently enrolled in the program. The Licensing Inspector observed 19 children in attendance at the time of the inspection. There were 2 staff members actively supervising the children's activities. A third staff member was also present whom prepared meals and provided classroom support as needed. An administrative staff member (FDS) was also present during a portion of the inspection. The children were served breakfast. The LI observed the children during the opening morning circle activity. Staff was also observed reading to the children. Other observed activities included: music, self-selected center activities, hand washing, the lunch meal and nap time. A sample of 5 children's records was reviewed. Documentation of the annual fire and health inspections was current. The first aid kit was observed in the center. Documentation of the practiced emergency drills was reviewed. Prescribed medications were observed in the center for two children. The Center was clean, orderly and supplied with age appropriate materials for the children. Staff were engaged with the children and provided guidance as needed. Findings were reviewed with the Family Development Specialist at exit. Standard #: 22VAC40-185-140-A
Description: Based on a sample review of children's records, the Center failed to ensure documentation of a physical examination by or under the direction of a physician was obtained for each child. Evidence: Documentation of a physical examination was not available for review in one of five reviewed children's records (child #1, entry date listed as 08/13/18) The physical exam for child #4 was not obtained within the prescribed time frame, before the child's attendance or within 30 days of attendance. The child's entry date was noted as 09/28/18; the PE was dated 01/29/19.

Plan of Correction: Staff will follow center's procedures to ensure PE documentation is obtained within the required time frame. A training is scheduled to review center policies with responsible staff to address enrollment practices to ensure compliance.

Standard #: 22VAC40-185-60-A
Description: Based on a sample review of 5 children's records, the Center failed to ensure the following information was complete: 60.A.2.: name, home address , and home phone number of each custodial parent; 60.A.3.: when applicable, work phone number and place of employment for each custodial parent. Evidence: One of five reviewed records was not complete, the record for child #4 indicated legal joint custody between the parents, information was provided for only one parent.

Plan of Correction: Staff will review the record with the parent and will request complete information, complete documentation will be retained in each child's record.

Standard #: 22VAC40-185-550-M
Description: Based on a sample review of minor injury reports, the Center failed to ensure the following information was maintained on a written record: 550.M.5.; date and time when parents were notified. Evidence: A written record of a minor injury on 11/28/18 was not complete with the time the parent was notified of the injury, a child was using scissors and cut skin on finger.

Plan of Correction: Staff will be reminded to follow center's policy and document complete information on all injury reports as applicable.
