Technical Assistance:
Changing children's pull-ups was discussed with the program director. Soiled pull-ups must be changed on the changing station and disposed in a disposal unit that is leak-proof and not hand operated. The local health department must be contacted annually to conduct an environmental health inspection.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 9:05 AM and departed at 12:20 PM. Nine children were in care with two staff at the beginning of the inspection and four staff at the end of the inspection. Five staff records and a sample size of five children's records were reviewed. Injury records were reviewed. The facility does not administer medication to children in care. An exit meeting was conducted with administrators prior to closure of the inspection.

Standard #: 22VAC40-185-60-A
Description: Based upon review of five children's records, the facility has not ensured that the record for each child includes the complete street addresses of two persons to be contacted in an emergency when a parent cannot be reached. Evidence: 1. The record provided for child 3 did not include the cities and states or zip codes of the two listed persons to be contacted in an emergency when a parent cannot be reached. 2. Staff 2 verified that the record for child 3 did not include the cities/states or zip codes for the two emergency contact persons.

Plan of Correction: The facility responded with the following: All children's records will be checked to ensure that complete street addresses are included for the two designated emergency contact persons.

Standard #: 22VAC40-185-500-A
Description: Based upon observation, the facility has not ensured that children wash their hands with soap and running water or disposable wipes before and after eating meals and snacks. Evidence: The children did not wash their hands after eating morning snack. The children went from the snack table directly to putting on coats for outdoor play.

Plan of Correction: The facility responded with the following: Staff will ensure that children wash their hands before and directly after eating meals and snacks.

Description: Based upon review of the records provided and staff interview, the facility has not ensured that they have implemented monthly practice evacuation drills. Evidence: 1. The records provided di not include documentation of an evacuation drill in September 2018 or in January 2019. 2. Staff 1 verified that the records did not include documentation of drills conducted in September 2018 or January 2019.

Plan of Correction: The facility responded with the following: Administrators will ensure that evacuation drills are conducted and documented monthly.
