An unannounced monitoring inspection was conducted on February 26, 2019, in regards to a child's injury. Staff interviews were conducted, video footage was reviewed, observations were completed at the facility , documentation was reviewed and obtained. Violation has been cited related to the information submitted by the licensee of a report of an injury to a child while under the supervision of a center . Standard #: 22VAC40-185-340-A Description: Based on review of video footage and staff interviews, the licensee did not ensure care, protection and guidance. Evidence: The video footage reviewed from February 8, 2019, on the playground, there were children from ages 16 months - 22 months on a outdoor play equipment with a slide, ladder, steps and rock climbing walls and a look out platform. There were children observed on the video footage falling off the platform on February 8. The manufacture age for the equipment was for 2 years -6 years. The center did not follow manufacturing instructions. Plan of Correction: All staff will be trained on or before March, 8, 2019, on ensuring that no child under the age of two is allowed access to this piece of play equipment. Management will observe daily for compliance and this will become a part of all new hire training as well.
