- Comments:

The licensing inspector initiated an unannounced complaint inspection in the center on February 28, 2019, continued on March 4, 2019 and completed on March 6, 2019 in response to a complaint received by the licensing office regarding special services, programs and physical plant. Review of center documentation, interviews with staff members and observations were conducted. Based on information gathered during the investigation, a preponderance of evidence does support the allegation regarding Special Services, therefore the complaint is determined to be VALID.

Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observation and staff interviews, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: During the inspection on March 4, 2019 the refrigerator approximately 4 feet 8 inches tall was sitting on a counter that is approximately 26 inches high. The refrigerator was not attached or secured to the wall and easily moves and is wobbling. This poses a tipping hazard. Plan of Correction: The Center Director had the refrigerator mounted to the wall.
Standard #: 22VAC40-185-560-K Complaint related: Yes Description: Based on observation and staff interviews, the licensee did not ensure tables and high chair trays were sanitized before and after each use for feeding. Evidence: During the inspection on March 4, 2019, child #1 was observed during feeding and the highchair was not sanitized before or after feeding. After child #1 was observed eating a center staff member sprayed a sanitizing solution onto a towel and then wiped off highchair tray. The highchair tray was not sanitized as required. The definition of sanitizing means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution. Plan of Correction: The Center Director will retrain the staff on the proper sanitizing policies and procedures.
Standard #: 22VAC40-185-570-C Complaint related: No Description: Based on record review and staff interviews, the licensee did not ensure the record of each child on formula contained all required information. Evidence: During the inspection on March 4, 2019, five out of ten children on formula did not have record that contained the brand of formula or feeding schedule. Classroom teachers and Center Assistant Director confirmed this during inspection. Plan of Correction: The Center Director had forms updated and new parents filled out forms for all infants enrolled in the class.
