An unannounced monitoring inspection was completed on 3/12/19. There were 44 children present with seven staff supervising. Six children's records were reviewed. Seven staff records had been previously reviewed in February 2019. Five medications were reviewed. The inspector arrived at the school at 2:30 pm and departed at 5:00 pm. The children were observed at arrival, during snack, during outside time, and during departures. There was discussion with the site director about the following: fire inspection due this month, "Report of Environmental Sanitation Inspection" form, having complete emergency contact addresses for all children's emergency contacts, documenting first dates and last dates of attendance, number of staff having first aid and CPR certification, practicing two shelter-in-place drills each year.

Standard #: 22VAC40-185-140-A Description: Based on record review, the center failed to ensure that each child had a physical examination by or under the direction of a physician within 30 days of enrollment. Evidence: 1. There was no physical in the record for child #7. Child #7 first date of attendance was 10/16/18. Plan of Correction: The site director will remind the child's guardian to get a copy of the child's most recent physical exam.
Standard #: 22VAC40-185-510-E Description: Based on observation, the center failed to ensure that medication was labeled with the the dosage amount and time/times to be given. Evidence: 1. A liquid antihistamine was observed for a child in care. The medication was not labeled with the dosage amount or time/times to be given. The medication had been given during this school year. Plan of Correction: The site director will label the medication.
Standard #: 22VAC40-185-510-G Description: Based on observation and interview, the center failed to ensure that prescription medication was stored as required. Evidence: 1. There was an inhaler observed for a child in care that did not have the original labeled container from the pharmacy with the pharmacy label attached. The inhaler was observed in no box and there was no prescription label for the medication. Plan of Correction: The site director will ask the guardian to provide the box and pharmacy label.
Standard #: 22VAC40-185-510-N Description: Based on record review and interview, the center failed to ensure that when an authorization for medication expires, the parent was notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. Evidence: 1. There was a medication (Epipen) for a child observed at the center that expired October 2018. According to the site director, the child no longer attends the program. The site director stated that the child left the program some time at the end of November 2018. The medication was still at the center after 14 days when the child was disenrolled and the authorization was no longer effective. Plan of Correction: The site director will send the medication to the parent.
Standard #: 22VAC40-185-560-F Description: Based on observation of the posted menu, the center failed to ensure that substituted food was documented for snack provided by the center. Evidence: 1. The menu for the date of the inspection had teddy grahams and applesauce listed. Corn flake cereal and pineapple was actually served. The menu was not changed to show the substituted food. Plan of Correction: This was corrected during the inspection. The site director will ensure that the menu is accurate for each day and note substitutions on the menu.
