22VAC40-185 STAFFING AND SUPERVISION.
22VAC40-185 SPECIAL SERVICES.
An unannounced monitoring inspection was conducted on 1/16/19 from 12:00 - 2:00 pm to investigate a self-reported incident regarding supervision of children and transportation. Observations and staff interviews were conducted. Staff records, attendance sheets, the parent handbook and center policies and procedures regarding transportation and safety were reviewed. A violation notice was issued as a result of this inspection. An exit interview was conducted on 3/06/19. Standard #: 22VAC40-185-40-E Description: Based on review of policies and procedures, interviews with staff, and review of documentation, it was determined that the facility failed to maintain compliance with their own policies and procedures regarding transportation, as required by the standards. Evidence: on January 7, 2019, Child A, age five, was left alone on the facility's bus from 4:22 - 4:55 pm. The facility's "Bus Safety Procedures - Returning to the School" require that staff 1) Unload children using the bus roster and face-to-name transition sheet, 2) Use the weekly center in/out form to verify attendance, and 3) Return to the bus; walk to the back of the bus and ensure no children or personal belongings are left on the bus. Staff A failed to follow these procedures resulting in a child being left unattended on the bus for 33 minutes. Plan of Correction: The Transportation policy will be fully implemented and enforced. All staff responsible for transporting children including management will be retrained on the transportation policy and procedures immediately. Completed 01/08/2019
Standard #: 22VAC40-185-340-A Description: Based on interviews with staff, and review of documentation, the facility failed to ensure the care, protection and guidance of children in care. Evidence: 1) on January 7, 2019, Child A, age five, was left alone and unsupervised on the facility's bus from 4:22 - 4:55 pm. 2) Staff A and Staff B were unaware that the child was not present in the school age classroom for approximately 33 minutes. 3) at 4:55 pm Staff B alerted management that Child A was not in the school age classroom and, along with management, found the child alone on the facility's bus parked in the parking lot. The temperature at that time was approximately 40 degrees. The child, who had just woken up, was crying when found by staff. Plan of Correction: The Bus Safety procedures for unloading at the school will be fully implemented and enforced for arriving at the school, unloading and once inside the school. All staff responsible for transporting children including management will be retrained on the Bus Safety Procedures immediately. Completed 01/08/2019
Standard #: 22VAC40-185-340-F Description: Based on interviews with staff, and review of documentation, the facility failed to ensure a child under 10 years of age was always within actual sight and sound supervision of staff. Evidence: on January 7, 2019, Child A, age five, was left alone on the facility's bus parked in their parking lot from 4:22 ? 4:55 pm. The bus was parked by the playground, and could not be seen from inside the center. Plan of Correction: The Child Supervision policy will be fully implemented and enforced. All staff responsible for transporting children including management will be retrained on the child supervision policy and procedures immediately. Completed 01/08/2019 All other staff will be retrained on Child Supervision Policy in the March Staff Meeting 03/21/2019
