There is one AuviQ medication on site. It is highly recommended that at least one staff person complete the specific AuviQ Medication Administration Training. The Center Director was not present during this inspection. Today's exit interview was conducted with the office Administrative Assistant.

An unannounced monitoring inspection was conducted today. There were 85 children present, with a total of 12 staff members assigned to their direct supervision. Children's records, staff training, background checks, health records, the physical plant, emergency preparedness items, and children's medications were inspected today. The children were observed playing outside, creating rainbow art projects, painting pictures, attending science class, playing with rainbow magnets, eating snack, and enjoying many various creative play activities. This inspection was completed between approximately 10:00am and 12:00pm. Thank you for meeting with me today. Standard #: 22VAC40-185-330-B Description: Based on observations, it was determined that various fall zone areas of the playground do not have an adequate level of resilient surfacing. Evidence: There is not enough mulch at the bottom of the slide and beneath the two bar climbers. In these areas, the mulch has worn down to dirt. Plan of Correction: We have already ordered mulch and it is scheduled to be delivered at the end of the month. We will rake the mulch from other areas of the playground until the additional mulch is delivered and spread where needed.
Standard #: 22VAC40-185-350-E-4 Description: Based on observations, it was determined that one classroom of children between the age of 3 to the age of school eligibility was briefly operated beyond the allowable ratio of one staff member for every 10 children. Evidence: The children in the Pups classroom are age 4 and 5. The Inspector observed this group of children proceeding from the classroom to go upstairs for Science class. During this brief transition from downstairs to upstairs, one staff person was alone with a 14 children. Plan of Correction: The second staff person had to stay behind with one child to deal with a special issue. All staff will be reminded to call for the floater or an office representative in order to always maintain ratio.
Standard #: 22VAC40-185-510-C Description: Based on a review of children's medications, it was determined that one authorization form has expired. Evidence: The authorization for one child's long term prescription medication expired on March 2, 2019. Plan of Correction: The parent will be asked to provide a new medication authorization form.
Standard #: 22VAC40-185-560-F-4 Description: Based on observations, it was determined that children under the age of 3 were offered a snack food that is considered to be a potential choking hazard. Evidence: The children in the Brown Bears classroom are age 2. The Inspector observed these children being served popcorn as part of their snack foods. Plan of Correction: Snack is provided for the class by a daily assigned parent. Parents will be notified not to provide popcorn for snack in any classroom.
