63.2(17) License & Registration Procedures

Your time and assistance were appreciated during the unannounced renewal inspection conducted on March 11, 2019, between the hours of approximately 9:45 a.m. and 1:25 p.m. Twenty-nine (29) children were enrolled and twenty-seven (27) children were in the care of three staff members during the on site inspection. Five children's records and five staff records were reviewed and/or updated. Two children's medications, as well as accompanying authorization forms, were reviewed. Posted information (license, emergency diagrams/plan, menu, emergency telephone numbers, asbestos information and daily schedules) and facility documents (attendance records, emergency evacuation and shelter-in-place drill records and one local agency inspection report) were observed/reviewed. The posted menu and daily schedules accurately reflected the snack served and the activities provided. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities observed on this date included: indoor play (coloring sheets, toys and manipulatives); grouptime (songs, numbers/counting, weather and fingerplay); stories; restroom and handwashing procedures; snack; outdoor play and departure of children. Staff/child interactions, behavior guidance and daily routines/transitions were observed. Staff members were observed providing attentive supervision during activities/transitions and meeting the individual needs of the children in care. Refer to the accompanying violation notice for current exceptions to standards compliance. The following areas of the licensing standards were discussed: medication procedures and complete documentation in child and staff records. Standard #: 22VAC40-185-510-C
Description: Based on observation of medications and discussion with staff on March 11, 2019, the program failed to ensure procedures were in place to prevent the use of outdated medication. Evidence: 1. Observation and review of two children's medications, on this date, determined that one of child F's medications was outdated. The medication was dated February 2019. 2. Staff confirmed the date of the expired prescription medication.

Plan of Correction: The child's parent was contacted and an updated prescription medication will be obtained. In the future, medications will be reviewed to ensure outdated medications are sent home/replaced.

Standard #: 22VAC40-185-510-J
Description: Based on observation/review of two children's medications on March 11, 2019 and discussion with staff, the program failed to ensure medications were kept in a locked place using a safe locking method that prevented access by children. Evidence: 1. Observation/review of medications for children F and G, on this date, determined that the medications were stored on high shelves in classroom cabinets. 2. Staff confirmed that the medications were stored out of children's reach but not kept in a locked place.

Plan of Correction: Medications will be relocated to locked areas in the classrooms. Administrative staff will monitor classrooms to ensure medications are stored in a locked place in each classroom.

Standard #: 63.2(17)-1720.1-A
Description: Based on review and/or update of five staff records and discussion with staff on March 11, 2019, the program failed to ensure fingerprint results were obtained for one employee prior to employment. Evidence: 1. Review of staff #4's record, on this date, determined that fingerprint results were not obtained prior to employment (date of employment - 2/13/2019 and date of fingerprint results - 2/14/2019). 2. Administrative staff verified the dates of employment and fingerprint results.

Plan of Correction: In the future, fingerprint results will be obtained prior to employment for newly hired staff members.
