Technical Assistance:
The classroom located adjacent to the front preschool classroom was reviewed and measured. Staff must maintain sight and sound supervision of children when this classroom is in use. Standard 340.F and Standard 340.A discussed Standard 60.A.5 reviewed - emergency contacts for children Discussed placement of lamp in the middle preschool classroom

An unannounced monitoring inspection was conducted on 02/27/2019 from 8:55am - 11:25am. There were a total of 12 children in care with four staff. The children ranged in age from infant - four years. Children were observed during morning program time and lunch. Nap was observed in the infant classroom. Records were reviewed for seven children and five staff. The director reports no medication is being administered at this time. The violations are listed on the violation notice and were reviewed with the program director at the exit interview.

Description: Based on record review, the center failed to ensure that the results of the required physical examination were obtained Evidence: 1. The documented physical on file for child 1 was not completed. The form was signed and dated 08/23/2018 however, none of the required information was completed on the form therefore, the results of the physical exam are not on file. a. Child 1 has an enrollment date of 08/20/2018.

Plan of Correction: Will be disputed. There is a physical file for child 1, however, the dr. did not complete the entire form.

Description: Based on record review, the center failed to ensure that children's records contained all of the required elements. Evidence: The record for child 2 lacked two designated emergency contacts.

Plan of Correction: Files will be updated immediately.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center shall be maintained in a safe condition. Evidence: 1. Cords attached to a telephone and echo speaker were observed within reach of children in the middle preschool class. These devices were placed on a shelf in this room that was accessible to the children in care. a. Dangling cords are an entanglement hazard. 2. A flat screen television was placed on a shelf in classroom 5. The television was not secured to the wall or media stand in any way. a. This television was within easy reach of the children in the classroom and is a toppling hazard.

Plan of Correction: 1. Cords have been secured to the wall. 2. Will be disputed as the TV is less than 5 lbs and only brought out for nap.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. Evidence: One outlet, located on the upper area of the wall, in the middle preschool classroom lacked protective covers.

Plan of Correction: Will be disputed as this particular outlet is 5 ft up the wall and out of reach of the children. Outlet has been covered, but was never a threat to children.

Description: Based on record review, the center failed to ensure that the written record of children's serious and minor injuries shall include all of the required elements. Evidence: 1. A review of 14 injury reports found 13 lacking documentation of any future action to prevent recurrence of the injury. 2. A review of 14 injury reports found 14 lacking documentation of the time the parent was notified of the injury.

Plan of Correction: Will ensure all accident reports are completely filled out.
