Comments:
The licensing inspector conducted an unannounced complaint inspection on 3/4/19, 3/6/19, 3/8/19, and 3/12/19 in response to a complaint received by our department on 2/8/19. The allegations of the complaint were in reference to ensuring the care and protection of children, the supervision of children, staff training, behavioral guidance, and parental involvement. Classroom observations were made, interviews were conducted, and records were reviewed. The preponderance of the evidence gathered during the investigation supports one or more of the allegations and the complaint is determined valid. See the violation notice for more detail. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Complaint related: No
Description: Based on review of four staff records, one record did not contain a current tuberculosis screening. Evidence: The record for Staff #2 (hire date 10/17/16) contained a tuberculosis screening dated 10/25/16. Staff are required to resubmit tuberculosis screening results every two years.

Description: Based on review of four staff records and interview, two records did not meet the qualifications for program leader. Evidence: The record for Staff #2 (hire date 10/17/16) and Staff #4 (hire date 1/7/19) did not contain documentation for qualifications for the position of program leader. Program leaders may qualify with six months of supervised programmatic experience if within six months before being promoted or beginning work or one month after being promoted or beginning work, a minimum of 24 hours of training shall be received related to the care of children, including but not limited to child development, playground safety, health/safety issues, and preventing/reporting child abuse and neglect.

Description: Based on observation, record review, and interview, the center did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Evidence: 1. On 3/4/19, the licensing inspector observed a ratio of 1:2 in the school age room. The teacher present (Staff #3) reports being a class aide. Administration reported that Staff #1 who was working at the front desk, was acting the lead for this classroom until Staff #4 returned from the bus runs. Although Staff #1 qualifies as a lead teacher based on a record review conducted 3/6/19, the licensing inspector observed that Staff #1 was additionally tasked with handling walk-ins, phone calls, and assisting teachers as needed in the building. The licensing inspector observed that Staff #3 was directly supervising the children inside the classroom on 3/4/19. 2. On 3/6/19, the licensing inspector observed a ratio of 2: 23 in the school age classroom. Administration identified Staff #4 as the lead teacher for the school age classroom and Staff #3 as an aide. The licensing inspector observed the record for Staff #4 (hire date 1/7/19) did not contain qualifications for the position of a program leader.

Description: Based on a review of four staff records, the licensee failed to ensure that two staff had central registry findings within 30 days of employment. Evidence: 1. The record for Staff #3 indicated a hire date of 1/27/19 and there was no no central registry finding recorded. Staff #3's record had documentation that the central registry search request was mailed on 2/1/19. The record did not contain documentation of any further contact, and the staff is still employed. 2. The record for Staff #4 indicated a hire date of 1/7/19 and there was no no central registry finding recorded. Staff #1's record had documentation that the central registry search request was mailed on 2/1/19. The record did not contain documentation of any further contact, and the staff has been continuously employed.
