Today an unannounced Monitoring Inspection took place between the hours of 10:40am and 2:00pm. At the start of the inspection there were a total of 58 children with 11 staff; all ratios were incompliance. A total of 5 children?s files and 5 staff files were reviewed. The children were observed working on a painting craft, being read to and playing in the indoor play room. Five medications and authorizations were reviewed. Standard #: 22VAC40-185-160-C Description: Based on record review, staff did not resubmit the results of a follow-up tuberculosis (TB) every two years. Evidence: Staff #5's most recent TB screening is dated 7/12/2016. Plan of Correction: If an updated screening was not done, we'll get a new screening scheduled today.
Standard #: 22VAC40-185-60-A Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 record did not contain the parents work contact information, addresses for either emergency contacts or the child's first date of attendance. Plan of Correction: I'll call the family today as well as make a note in their tadpole account that we need an update.
Standard #: 22VAC40-185-70-A Description: Based on record review, staff records did not contain all required information. Evidence: Staff #1 file does not contain an address for their emergency contact or documentation of orientation. Staff #4 file does not contain documentation of two references. Plan of Correction: I'll reach out to the staff to let them know of the missing items.
Standard #: 22VAC40-185-280-D Description: Based on observation, cleaning materials were stored above food and single-service articles. Evidence: Liquid hand soap was above an open bag of pretzels and paper plates in the cabinet's over the counter in the Preschool 1A classroom. Plan of Correction: This was corrected during the inspection.
Standard #: 22VAC40-185-510-E Description: Based on observation, medications were not labeled with the child's name. Evidence: Child #2 and Child #6's emergency medication's were not labeled with the children's names. Plan of Correction: This was corrected during the inspection.
