An unannounced monitoring inspection was completed on 03/22/2019. Present during the inspection were 8 children, ages 3 years-5 years, in the direct care and supervision of two staff members. There were additional support staff members also on-site during the inspection. The inspector reviewed 5 children's records and 2 staff records during the inspection. The staff stated that there is no medication being administered at this time. The children were observed in the following activities: various learning activities, large motor play in the gym, reading and during morning snack. The inspector reviewed the emergency evacuation plan, the shelter-in-place plan and the emergency supplies during the inspection. The inspector discussed children's records, the storage of supplies and sanitization with the staff. Standard #: 22VAC40-185-240-D-6 Description: Based on record review and discussion with the staff, the facility failed to ensure that there shall always be at least one staff member on duty who has obtained within the last three years in performing the daily health observations of children. Evidence: The most recent daily health observations course documented in the record for Staff #1 was dated 09/09/2015. There was not an updated course in 2018. Plan of Correction: A staff member will be trained in the daily health observation.
Standard #: 22VAC40-185-290-3 Description: Based on observations and discussion with the staff, the facility failed to ensure that electrical outlets shall have protective covers in areas used by children preschool-aged and younger. Evidence: 1. There were 6 uncovered electrical outlets in the gym; the children were observed playing in the gym during the 03/22/2019 inspection. 2. There was an uncovered electrical outlet in the preschool classroom; the children were observed in the classroom during the 03/22/2019 inspection. Plan of Correction: The staff will check before going into the gym to make sure outlets are covered.
