An unannounced Monitoring Inspection was conducted today from 7:45am to 9:10am. There were 23 school-agers directly supervised by 3 staff. The physical plant, 3 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing freely and going to wash hands for breakfast. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. None of the staff on site had current DHO training. Staff-to-child ratios were in compliance. Standard #: 22VAC40-185-240-D-5 Description: Based on review, the facility did not have at least one staff member on duty who has obtained within the last 3 years instruction in performing the daily health observation of children. Evidence: 2 staff were on site, and neither had documentation of Daily Health Observation completed within the last 3 years. Staff 2's last daily health training was dated on 8/15/15. Plan of Correction: Staff will be trained in DHO in April.
Standard #: 22VAC40-185-350-E-5 Description: Based on observation, the facility did not maintain staff-to-child ratios of one staff member for every 18 children. Evidence: Observed that during the inspection, one staff member left the room, leaving the other staff member with 22 children. The staff member returned in approximately 3 minutes. Plan of Correction: Corrected at the time of the visit.
Standard #: 22VAC40-185-510-K Description: Based on observation, the facility did not ensure that when medication is locked, and a keys is used, that it is not accessible to the children. Evidence: Observed that the medication box has a lock with a key attached to the box on a string, and is kept on a lower shelf in a closet that was not locked during the inspection. Plan of Correction: Staff will come up with a new system.
