Comments:
A Subsidy Health and Safety inspection was conducted on 3/19/19 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 3/19/19. There were children present. Five children's records and five staff records were reviewed. No medications were at the center. The inspector arrived at the center at 3:10 pm and departed at 5:45 pm. The children were observed at arrival, during supper time, during homework time, and during gym time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on interview, the center failed to ensure that there was a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: 1. The program director stated that there has not been a policy developed for care by consistent staff or team of staff members.

Plan of Correction: The program director will add this to the staff handbook.

Standard #: 22VAC40-665-770-B-5
Description: Based on interview with the program director, the center failed to ensure that there was a written procedure for ensuring essential functions are maintained during an emergency (continuity of operations procedures). Evidence: 1. The program director verified that there was no written procedure for continuity of operations.

Plan of Correction: The program director will add this to the emergency plan.
