Comments:
An unannounced renewal inspection was conducted with the morning administrator and director beginning 7:15 AM and ending at 11:00 AM. There were 48 children in care during the inspection. The children's ages ranged from 7weeks through 9 years old. The children were observed listening to music, playing with toys, block, and manipulatives. Ten children's records and five staff records were reviewed and in compliance. Three medications was reviewed and in compliance. The first aid kit, flashlight, and weather-band radio were observed and in compliance. One bus was inspected and determined to be in compliance. The emergency escape drill log was reviewed and in compliance. The most recent emergency evacuation drill was documented as 3/11/19. The most recent shelter-in-place was documented as 2/6/19. The most recent health inspection was documented as 9/26/18. The most recent fire inspection was dated 6/6/18. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, not all of the areas and equipment on the outside of the center were not maintained in a safe and operable condition. Evidence: The key was left inside the electronic panic bar lock on the fence door located on the toddler playground. The electronic panic bar was not functioning properly as it was not able to be locked and the key removed without the alarm sounding.

Plan of Correction: There is a slide lock on the fence door, it was locked and secured during the inspection. The maintenance technician was contacted during the inspection regarding the electronic panic bar lock. The center will keep the children off the playground until the lock is fixed.

Standard #: 22VAC40-185-350-A
Description: Based on observation and staff interview, the center did not maintain the required staff-to-child ratio when directly supervising children. Evidence: On March 26, 2019, there were eleven three and four year old children in the care of one staff member. There was a two year old child that was unable to go into a classroom due to staff-to-child ratio from 7:15 AM to 7:20 AM.

Plan of Correction: The center will make changes to staff schedule to cover when children arrive to the center during morning hours.
