Thank you for your time and assistance with the unannounced complaint inspection conducted on March 25, 2019. The on-site inspection was held from 2:50 p.m. to 4:40 p.m. At the time of arrival there were 48 children in the care of 12 staff members with two additional staff members available to assist if needed. There were 112 children enrolled in the program. The inspection was in response to a complaint that was received in the Valley Licensing Office on March 22, 2019. The complaint was regarding Part V (Staffing and Supervision), Part VI (Programs) and Part VII (Special Care Provisions and Emergencies) of the licensing standards. The preponderance of the evidence gathered during the investigation did not support the allegation and the complaint disposition was, therefore, "not valid." There were no violations of the licensing standards cited. Three staff members were interviewed. Two staff members' records and one child's record were reviewed. If you have any questions, I may be contacted at (540) 280-0742.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information.
