An unannounced monitoring inspection was conducted at the facility on March 20, 2019 from approximately 9:07am-12:21pm. Upon the entrance there were approximately 90 children present with 15 staff members. Additional staff was on site for administrative support. The Licensing Inspectors observed children in circle time, exploring music, painting, eating snack, cleaning, coloring , writing, working with wood pieces and reading independently . The Licensing Inspectors reviewed 5 staff records and 6 children records. Measurements for a new classroom was completed. New toddler classroom had two bathrooms and the classroom was approximately 566 square foot. The exit interview was conducted with a administrator.

Description: Based on 1 of 5 staff records reviewed, the licensee did not ensure staff had documentation of tuberculosis screening within the past 2 years. Evidence: During the inspection conducted on March 20, 2019, staff #2 last T.B. test was completed on January 23, 2017, which is more than 2 years old.

Plan of Correction: Staff member completed T.B. screening.

Description: Based on review of attendance logs and interviews with staff, the center did not ensure to comply with the terms of the current license issued by the department. Evidence: During the inspection March 20, 2019, interviews with staff and observation a new area of the building not included in current license was in use by the staff and children in care prior to the modification submitted the licensing department.

Description: Based on staff interview and observation, the licensee did not ensure daily schedule was posted. Evidence: During the inspection conducted on March 20, 2019, in two of the three toddler classes there was not a posted daily schedule.

Description: Based on observation, the licensee did not ensure the diapering surface was used only for diapering or cleaning children. Evidence: During the inspection conducted on March 20, 2019, in the Toddler I classroom, there was a bowl with green rice on the changing table . In the new toddler classroom, there was a clipboard , notebooks and a other items being store on diaperig surface.

Description: Based on record review, the licensee did not ensure there was a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: During the inspection conducted on March 20, 2019, there was not documentation of two shelter in place drills completed in 2018.

Plan of Correction: Complete a shelter in place drill on 3/26/2019 and another in Sept. 2019.

Description: Based on 1 of 5 record review, the licensee did not ensure that staff who lived out of state in the past five years had a central registry completed in the state they live in the past five years. Evidence: On March 20, 2019, Staff #5( hire date 09/29/2018) sworn statement of affirmation documented that the person lived outside of Virginia in the past five years. The center confirmed they did not request a central registry from the state. During the inspection Staff #5 confirmed she lived outside of Virginia in the past five years.
