Comments:
An unannounced monitoring inspection was conducted from 1:00 - 3:00 pm with center staff. There were 31 children, ranging from three years to five years of age, present with six staff supervising. Children were observed napping during the inspection. Four children's files and two staff files were reviewed. The emergency escape drill log was observed and in compliance. The playground was observed and in compliance. The first aid kit, battery operated radio and flashlight were observed and in compliance. One emergency medication was reviewed and in compliance. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of two staff files, and an interview with staff, it was determined that repeat tuberculosis (TB) test/screenings were not completed every two years. Evidence: The TB test for staff A is dated: 9/30/16 and for staff B is dated: 3/14/16.

Plan of Correction: Staff have had repeat TB tests/screenings and will locate documentation and update files.

Standard #: 22VAC40-185-60-A
Description: Based on a review of four children's files, it was determined that not all files were complete. Evidence: The file for child #1 did not have a phone number for either of the two emergency contacts.

Plan of Correction: Will ask parents for the missing information.

Standard #: 22VAC40-185-450-A
Description: Based on observation, not all children had linens consisting of a top cover and a bottom cover during the designated rest time. Evidence: two children in one preschool room were observed resting on mats without a bottom or top cover.

Plan of Correction: The children had blankets that were placed on their cots during the inspection. Parents will be reminded to send in a bottom sheet as well.
