A Subsidy Health and Safety inspection was conducted on March 4, 2019 from 3:05 PM to 5:58 PM and at the office on March 6, 2019 from 10:10 AM to 11:25 AM to review supplemental health and safety requirements. Today, there were twenty-nine school age children who attended during after school hours with two to three staff. I viewed operational responsibilities, record keeping, reports, caregiver training, hazardous substances, behavioral guidance, emergency planning and drill records, food service, allergies, five records for children, and five staff records. A licensing inspection was conducted on the same dates as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Standard #: 22VAC40-665-540-A Description: Based on a review of five records for staff, and an interview with administrative staff, the program failed to obtain documentation of tuberculosis screenings for one staff prior to work with children and no earlier than thirty days before employment. Evidence: Staff member 3 began work with children by December 16, 2018 and the TB screening was not completed until March 2, 2019. Plan of Correction: Administrative staff will arrange for new staff to obtain TB screenings within thirty days prior to beginning employment and make sure the statements are in the staff file as required. The site program director is using a system for writing record dates and making sure the required records are obtained on time.
