The licensing inspector conducted a Subsidy Health and Safety inspection Tuesday, March 26, 2019 from 10:20 am to 2:45 pm to review supplemental health and safety requirements. The children were observed engaged in a variety of activities. The infants were observed sleeping in cribs and playing with toys on the floor with their teachers. Ten children?s records and Ten employee records were reviewed during this inspection. Medication is being administered, however none was present The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/06/2019 Last shelter-in-place drill: 02/19/2019 Last lockdown drill: 11/12/2018 Total enrollment: 106 Today, the following child to staff ratios were observed: Infants I ? 15:4 Infants II-8:2 Toddlers ?19:4 Early Preschool (Twos) ? 26:4 Threes ?20:2 Pre-K ?18:2 School Age ?not present A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety requirements were found and are identified on the violation notice. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on observation and staff interview on 03/26/2019 the center did not ensure that group size requirements are maintained at all times. EVIDENCE: 1. The group size in the Infant I room (4weeks to 12 months old) was 15 infants. 2. The group size in the Toddler room (15 months to 24 months old) was 19. 3. The group size in the Early Preschool room (2 years old) was 26. 4. Staff #11 stated that each of the above classrooms was considered one group.
