An investigation was initiated on 2/28/19 in response to a complaint received by the Eastern Regional Office that same day regarding supervision. An unannounced on-site inspection was conducted on 3/1/19 and interviews with staff were completed. Records and documentation related to the incident were also reviewed. The investigation was concluded on 3/15/19. Based on information gathered during the investigation this complaint is determined to be not valid.
