Technical Assistance:
A discussion was had concerning playground safety and what procedures were in place.

Comments:
An unannounced other-self reported inspection was conducted regarding concerns of leaving a child under the age of 10 unsupervised. Staff was interviewed and video footage was reviewed. Standard #: 22VAC40-185-340-F
Description: Based on interview with staff and review of video footage, a child under the age of 10 was not within actual sight and sound of staff for approximately 3 minutes. Evidence: Child #1, (3.5 years old), opened the door from the playground entering the hallway at 5:27pm on March 27, 2019. He proceeded down the hallway to the classroom, entered the bathroom and began removing the soiled clothes. A parent who saw the child in the classroom, then informed the Assistant Director of the situation.

Plan of Correction: The playground safety rules were reviewed with all staff, reminding them they must be stationed all around the playground, including one must stay by the door at all times.
