Standard #: 22VAC40-185-140-A Description: Based on review of five children's records and confirmation from the director, one child's record did not contain a physical examination report. Evidence: 1. The record of Child #1 did not contain documentation that a physical examination had been obtained. Date of enrollment for Child #1 was 01/02/2019. Plan of Correction: The director will speak with parent of the child today and will obtain a physical exam report as soon as possible.
Standard #: 22VAC40-185-240-C Description: Based on review of two staff records and discussion with the director, one staff had not obtained the required number of annual staff development activities. Evidence: 1. The record of Staff #1 documented that she had obtained 12 of the required 16 hours of annual training from March 2018 through February 2019. Plan of Correction: Staff #1 will obtain four more annual training hours for the 2018-19 year and then 16 training hours from March 2019 to February 2020.
