An unannounced "focused" monitoring inspection was made on 4/3/19 in conjunction with the health inspector to follow up on the violation from the complaint inspection dated 3/4/19. The areas of the facility were observed and the violation from the complaint inspection was determined to be corrected at the time of this inspection.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and interview, the center failed to ensure that the findings from the most recent inspection of the facility were posted. Evidence: 1. The "Violation Notice" that would report the findings from the most recent inspection at the facility was not observed at the facility. 2. The office administrator was asked about the posting of the "Violation Notice" and was not able to locate the document posted at the facility. 3. There were two inspections, a monitoring and a complaint, that resulted in violations that were both completed on 3/4/19. Neither were posted at the facility.

Plan of Correction: This was corrected during the inspection. The "Violation Notices" were posted.
