Comments:
An unannounced mandatory inspection was conducted today between 12:15pm and 2:20pm. There were 35 children present with 9 staff members, well within staffing guidelines. A complete inspection of the physical plant was conducted. In addition the children's/staff records, emergency supplies, emergency procedures, attendance, and evacuation procedures were reviewed. The children were observed playing in small groups and preparing for nap. Diapering was also observed. Areas of non compliance were cited on the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on children's record review, the center did not maintain and keep at the center a separate record for each child enrolled.

Evidence:
Child #1 and #3's files did not have on record a complete physical examination before the child's attendance or within one month after attendance.

Plan of Correction: We will maintain physical examinations and keep previous ones from the beginning of the school year

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) In room 104 Lysol was not in a locked place using a safe locking method that prevented access by children. 2) In the toddler room, Hand sanitizer was not in a locked place using a safe locking method that prevented access by children.

Plan of Correction: We will make sure to keep chemicals out of the classrooms

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center did not ensure that electrical outlets would have protective covers that are of a size that cannot be swallowed by children. Evidence: In the toddler room, there were three outlets missing protective covers.

Plan of Correction: We will replace the covers.
